We also have supported permanent housing including a 23 bed Assisted Living Program targeting persons with special needs. Our youth programs include the Transitional Age Program (TAP) for homeless youth, and Youth Community Support. TAP helps homeless youth with mental illness, ages 16 to 24, learn the independent living skills necessary to transition to living on their own in the community within 24 months.
Adult Community Support Services include homeless prevention/intervention, case management, community stabilization services, and Projects for Assistance in Transition from Homelessness (PATH). The SOAR program helps homeless/near homeless persons with mental disabilities apply for and obtain SSI/ SSDI benefits as a stepping stone out of homelessness
